Personnel Files, Faculty

Contents of the official faculty personnel files are retained centrally by the Office of the Senior Vice President and Provost. These official faculty personnel files may include:

- Discipline letters (and/or other concluding document(s))
- Form I-9
- Letter of Intent/Contract
- Letters of recognition and/or commendations
- Old Forms 111
- Personnel Information Form
- Resume/Curriculum Vita

Note: Faculty annual evaluations are retained in the departmental personnel files as long as the faculty member is a current employee (until resignation/retirement occurs).
